Abstract

Transportation is very important thing to facilitate someone in moving to one place or region to another area. Transportation is accompanied by an accident so that the NTSC conducts an analysis of the causes and consequences of aircraft accidents. In this study, the researcher discusses the causes and consequences of aircraft accidents in a serious incident because these categories are the most recommended for analysis by the NTSC. The method used in this research is ORLIO. The method explains several analyzes, specifically Organization Influences, Risk Controls, Local Conditions, Individual Action and Technical Failure Mechanisms, and Occurrence Events where the interrelated analysis. The method is used to determine the causal flow of a serious incident that can occur because there are parameters and variables that support the process of analysis. Some steps are reading the full report, grouping the variables related to the cause and effect of the plane crash. Where these variables are grouped according to parameters. So the last step that can be done is to determine the flow of accidents can occur. In the 10 full report data analysis the causes of accidents that often occur are miss-understanding coordination caused by wind changes
